That's exactly why we created Evergreen's Pathways of Care.

  
Instead of asking you to choose a practitioner or service before you understand what may be contributing to how you're feeling, our Pathways help you begin with something much simpler:

But that's not necessarily how patients experience their health.

You don't wake up thinking, “I have a chiropractic problem.”

         

Maybe you've found yourself thinking:

I'm anxious and I can't sleep.
My body hasn't felt the same since having my baby.
I'm in pain and I don't know why.

What Are Evergreen's Pathways of Care?

Our Pathways of Care organize our services around what you're experiencing and what you want help with, rather than expecting you to already know which practitioner you need.

Each pathway brings together practitioners and services that may be helpful for that particular area of health.

Our online Find Your Pathway tool asks a few simple questions about what you're experiencing, what may be contributing and what feels most important to you right now. From there, it helps guide you toward a pathway and gives you options for where to begin.

At Evergreen, we believe physical and mental wellness are deeply connected.

One symptom doesn't always have one contributing factor.

That doesn't mean every patient needs every practitioner.

It means we want to look at the whole person before deciding what support makes sense.


What Collaborative Care Actually Means at Evergreen

   
Collaborative care is more than having several healthcare professionals working under the same roof.

For us, it means recognizing when another perspective may be valuable.

It might mean your massage therapist notices that strengthening and rehabilitation could be an important part of your recovery and recommends chiropractic care.

Your chiropractor may recognize that stress and sleep are playing a significant role in persistent tension and suggest naturopathic or mental health support.

A therapist supporting someone through postpartum anxiety may recognize that physical recovery is also affecting how that person feels day to day.

 

With your consent, our practitioners can communicate and work together so your care feels connected rather than fragmented.

And sometimes collaboration also means recognizing that one practitioner is all you need.

The goal isn't to give you more appointments.

The goal is to help you find the right care.
